About a decade ago, for Lem Decker when his health required, I modified a 
commercial ICOM headset/boom mike assembly for a talkie to work with a fixed 
rig.  It included VOX operation so no switches were required, but the VOX was 
switchable and the unit could also be used as PTT.

Article was written up in QST.  I'll see if I can find it.
